Parents and students may now complete registration ONLINE.
If you complete ONLINE registration before the registration dates all you will have to do on the registration days is pick up your computer and class schedules. You MAY NOT pick up computers and class schedules until the registration dates (click here for dates).

ALL forms should be submitted online and are required to be enrolled in Lincoln High School and to participate in any athletics/extracurricular activities or to drive and park a vehicle on the High School Campus.
A physical is also required for all athletics and can be found under the Download and Print TAB. Fill out the first page of the Physical, sign it and take to your physician. Once complete, turn Both the history and the physical form in to the athletic department. Physicals are valid for one year from date of Doctor’s appointment.
1. Create a login account for your household, then add each student. By doing this it will ensure that your information will be there the next time you have to go in and fill out forms each year. The information asked for will have to be entered only this year for each student and the next year it will recall all information and you will only have to update from this year forward.
2. To access the online forms, click on the Registration TAB. You will see a list of required forms to read and consent or fill out electronically. Please read each form carefully.
3. To sign the document, click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over, click on the refresh icon next to the signature box.
4. Once you have filled out each form and signed at the bottom, click submit. You will have the opportunity to print that page.
5. Enter your email address in the box provided and you will also receive a confirmation email. If you do not have an email you can skip this step.
6. ALL BLANKS MUST HAVE AT LEAST A (0) or NA in it in order to submit.
